Epic discharge indicators change to plain language
All Yale New Haven Health clinical locations are now using plain-language descriptions for patient discharge readiness categories in Epic. The change replaces the red, yellow and green readiness categories with simple text and associated red, purple and green indicators for patients’ discharge status. The new indicators are:
- Not medically ready for discharge (red - N)
- Medically ready, with barriers (purple - B)
- Medically ready, no barriers (green - R)
Care Management will use the new, plain-language indicators to document patient discharge readiness in Epic. The former Patient List Column D/C Status has been renamed Medically Ready for Discharge. This patient list column and Facility Boards have been updated to reflect the new documentation.
