Yung Ah Lee, MD

Yung Ah Lee MD is a geriatrician committed to improving care for older homebound patients as a part of the Yale New Haven Health Northeast Medical Group Geriatric Home Based Primary Care (HBPC). She specializes in providing home care for older patients who often have complex medical and social needs and yet have difficulty accessing usual healthcare due to being homebound. She and her team aim to help streamline healthcare, reduce unnecessary hospitalizations, and focus on outcomes that matter most to her patients and their families. She is affiliated with Yale Geriatrics and engaged in teaching learners of various levels. To be eligible for HBPC program, a patient must be (1) at age 65 or older, (2) insured, (3) generally homebound, (4) and live in following geographical areas: Bethany, Branford, East Haven, Guilford, Hamden, Madison, Milford, New Haven, North Branford, North Haven, Orange, West Haven, Woodbridge.
