Diabetes is a public health epidemic. More than 29 million Americans are living with the disease and an additional 86 million are at risk of developing diabetes. Diabetes is a leading cause of blindness, heart disease, amputation, end-stage kidney disease and liver problems — and the seventh leading cause of death in the United States.

That’s the bad news about this chronic disease, but thankfully there’s plenty of good news regarding the diagnosis, treatment, management and prevention of diabetes.

Our Approach to Diabetic Care

Northeast Medical Group endocrinologists are board certified leaders in the Yale New Haven Health network of physicians and hospitals, working together to help their patients live with diabetes. Diabetes care teams design personalized treatment plans, which may include using insulin or other medications, and often involve making lifestyle changes in diet and physical activity. Stressing preventive care, Northeast Medical Group practitioners monitor your health in order to avoid or minimize complications.

Basically, diabetes prevents your body from using the nutrients from the food you eat. There are two types of the disease.

Type 1 diabetes occurs when the pancreas produces little or no insulin, a hormone that moves blood sugar (glucose) through the bloodstream and into your body, using it for energy and growth. People with type 1 diabetes must take insulin, by injection or an electronic pump device. An incurable disease, type 1 diabetes accounts for less than 5% of diagnosed cases.

Type 2 diabetes occurs when the pancreas produces insulin, but the body doesn’t use it properly. You can manage type 2 diabetes with dietary and lifestyle changes, although medication also may be required. In fact, studies have shown that people can actually reverse type 2 diabetes by losing a large amount of excessive weight.

Northeast Medical Group endocrinologists partner with their patients and families to customize a treatment plan that fits each individual’s lifestyle. Plans include monitoring your blood sugar, taking medications, controlling your weight, eating a healthy diet, participating in physical activity and quitting smoking. Endocrinologists also coordinate with your primary care doctor and other specialists to reduce risks of or to treat complications.

Community Partners and Affiliates

The providers at Northeast Medical Group have developed integrated efforts with Yale New Haven Health - including Bridgeport Hospital, Greenwich Hospital, Lawrence + Memorial Hospital, Westerly Hospital and Yale New Haven Hospital - to help prevent diabetes as well as reduce progression of diabetes through:

  • Implementation of a diabetes registry, allowing Northeast Medical Group providers to identify patients with diabetes and reach out to them to help them manage their diabetes when we see that they have gaps in care
  • Care coordinators working with Northeast Medical Group primary care practices to provide additional education and support for patients with diabetes, ensuring they come to visits prepared and with up-to-date lab work, that they understand their medications, that they have access to the best information about diet and exercise to support improving their diabetes care
  • Close collaboration with Yale New Haven Health Certified Diabetes Educators, including a smooth, online referral process for those patients who wish to meet 1:1 with a diabetes educator
  • Partnerships and a common electronic medical record between primary care and endocrinology practitioners within Yale New Haven Health to be sure that all providers caring for our patients with diabetes have consistent and common information to have a consistent, common care plan and reduce the risk of unnecessary or redundant testing or intervention
  • Implementation of a “diabetes passport” – an individualized care plan with information for patients with diabetes to help them manage their own diabetes more successfully
  • Diabetes Self-Management Training (DSMT) and Medical Nutritional Therapy (MNT) referrals
